We had it the same way (US) until we changed the old cast iron pipes to PVC. It's just old metal piping that toilet paper sticks to.

Erm it's size of pipe not material. Russia, Turkey, Greece etc have smaller diameter pipes than say the UK or USA.

It's material as well, because old cast iron isn't as smooth as pvc, but I do agree bigger pipes are better to prevent clogs.
