Phototypesetting scrolls off the machine in big rolls called "galleys" 9 inches wide; stories, headlines, captions, everything that's in type form, and it's up to the people who actually cut out the various bits of type & paste them to the page to get it right. The graphics would come in from the stat-camera person (darkroom) separately. So it's real easy for this stuff to get swapped, especially if it's just after lunch when the paste-up crew just got done torching up a fat bomber in the alley.
