Depends how high off the ground it is. I think a harness is required for anything above 7 feet.
It's usually required at heights above 6 ft, but you still can't step on the top two steps of a ladder, that's a violation.

Exactly, they fine small company's out of existence, have seen it several times, safety is not their priority, control and revenue is.
They fine people for doing stupid shit, if a business goes out of business because of OSHA the owners were fucking idiots and hired idiots.
I'm gonna assume you don't live in the real world, if you did then you wouldn't make that statement. Sounds good on paper, but not reality
