I think it's more of a way of making sure they have the listeners complete attention. Especially over the phone.

It's quite the giveaway as a patient. You go from the initial "how am I doing" to "what is it? Give it to me straight."

It's a fairly common shock response when given severely bad news. Like "you basically have a death sentence" levels of bad news.

I decided when I was younger, if a Doctor ever tells me I have Cancer and have X amount of time to live i'll go "Promise?"
