There's normally a plastic cap on top. After removing that the opening is big enough to fit around your mouth, pull trigger, then inhale.

Anything over 10k puts people at risk of altitude sickness. The Grand is 14k and change IIRC, so not crazy, but you feel it at the summit.

Not really. Felt perfectly fine in the city. Higher up around Lookout Mountain, felt more tired/out of breath, but nothing serious
