You're describing a 2-stroke engine. In 4-stroke, valves let all combustion gas be blown out w/out unspent fuel. Cleaner + more efficient!

They've used a flat head because they can't be bothered making an ohv head from polycarbonate, which is fair enough.

Only uncommon in cars. This is an L head engine where the valves are inside the block. This is typically found in lawnmowers chainsaws etc.

The are very fuel inefficient (some fuel exits with exhaust) but very powerful (1 boom every up stroke vs every other up stroke).

2 Strokes have ports in the cylinder walls to draw in the fuel-oil-air mixture at piston bottom-dead-center. This is why they're 'dirty'.
