Weight is lost by spending more calories than you're taking in. So it's kind of both. Regardless, weight loss is a marathon, not a sprint. 10lbs in the last month is a great start.
i'm of the opinion that as long as you do your best every day that is good enough, and to remind yourself that some days you can't do that much and it is okay to take a break every now and then.
There is probably quite a bit of resistance training in positions he can do within his range of motion. And he probably has a trainer as well as a medical care person, on sight just in case? Idk. This dude is amazing. Yay him.
I have MS, lot of stuff I could do 10 years ago I can't do now, so I bit the bullet and hired a trainer to help me find lifts and modifications I could do. I bet he's got a pretty rock solid trainer too.

I was diagnosed about a year ago with MS, neurologist said from the MRI its something that started upwards of 10 years ago and I just finally had something trigger (half my face went numb, had muscle control but not sensation, heavy oral steroids regained sensation after a month). My biggest tells are aphasia and forgetting what i'm doing while i'm actively doing it, which some people would just call general to severe forgetfulness. We all show wildly differently.
My brother has it. It’s hell on earth for the ones that have it. And a nightmare for us all that have to watch him progressively get worse. It just depends on what stage they are on.
It progresses at different speeds in different people. Some can go decades with minimal impedement, and others can go from walking to wheelchair just months after diagnosis. And as you mention can have episodic ups and downs as your body attacks different areas.
