Nah, you can get spotted, get around them and put them to sleep. Or just run away to a roof or trash bin.
Putting them to sleep had might cause them to die to random blood flies or something else random. Can't remember how i got my achievement.

I have over 100 hours in both games. However, my first go is always high and takes around 20-30 hours (I'm an explorer). The second --
(no kills and never spotted) then takes me 10. I can do any run in around 6-8 now. Speed isn't the point though, it's about skillful play.
and taking time to plan the best route, then executing it flawlessly.
