This applies to all suit buttons. You must never do your bottom button up, it really annoys me when i see people do this, and all buttons should be undone if you are wearing a waistcoat.
You can fold your pocket square in may ways, but it should never match your tie only compliment it.
The shoes is where lots of people go wrong as they buy cheap shoes that look terrible. The shoes you buy should be 100% leather, brown/tan goes extremely well with blue suits.

If you ball out you can use whatever the fuck knot you want. I usually double-knot mine. Sorta like a shoelace.

I'd argue the half-windsor as the standard. Many folks don't purchase heavy/wide ties these days (the recommended material for a 4-in-hand).
