One of my DMs homebrewed a plane of existence that lies outside the "known" D&D planes called The Wanderlands. Where anyone from any plane, andly reality, or any planet could wake up one day and find themselves in it. Like...If it were a real place, you or I could end up there just as easily as a being from another planet in another galaxy in our reality. (She tied it in with the very first campaign we did, cuz we did some reality breaking shenanigans in it.)
