This happened last July in Traverse City Michigan at the annual Cherry Festival. No one was hurt, but nothing was ever officially reported on the cause....Though clearly the ride wasn't bolted down and secured properly. Props to the ride op that hit the emergency stop and ran to get help, and to the first guy who jumped on that railing breaking everyone's help paralysis.
Also, can we recognize the true hero in this situation: that fucking railing that managed to not rip right off with 20 folks literally holding down the ride hanging off of it with their full weight.

First it was last year, two no ride is bolted down, third a support pad slipped out from a ground support that levels the ride is all.

More then that, all rides have to be state inspected and tested on each setup. My place has had zero issues in 2 generations running it.
