This looks like a tail rotor drive shaft malfunction to me--the bird was in trouble before bits started breaking off. The main rotor generates lift, but generates torque and both act on the aiframe. To balance this, most helos use a tail rotor. About 10-ish percent of the torque generated by the engines goes to powering the tail rotor, but if that shaft were to snap, the 10% that was going towards countering main rotor torque now acts at the main rotor. You rise slightly, and then spin.

The crew chief does NOT sign for the aircraft, the PIC does. MX signs that the aircraft is safe for flight, but in no way are they deciding who can or cannot fly without stepping outside the bounds of their role.
