No, she was definitely conceptualized as a transgender woman. There was nothing after the fact about it.
It actually wasn't the Americans who made that choice. She was originally planned even in Japan to be transgender, or what they called "newhalf" as a slang at the time. It was definitely inspired by how they felt the global gaming audience would receive hitting a female character, but she was also planned that way from the very beginning for those reasons.
