In waymo it could be the software designer, if it's actually remote controlled then the controller, the manager who oversees when it happens, the alert system monitor/programmer and so many more issues. This is exactly why tech like this should require extreme oversight and occumpaning laws to handle such conditions

For an explanation generally a major leg in crime is intent, with your software you'd have directly told the software to do it. Before anyone says ultrasound tech isn't designed like that, you could custom make the device I'm speaking purely on its ability to see soft tissues under tissue. Or possibly are lumps too soft/small to normally be seen.
I mean wouldn't it be possible to do a course check regularly like an ultrasound or density? Surely a lump is mute dense than any of the surrounding tissues (not breast bone). If anything turns up then do a higher resolution scan. This would also help by not catching the random insedentalnomas as they're less likely to be seen in such a course scan.
