Are you sure it wasn't just roaming? Looking through the room one by one repeatedly. It’s a pretty common puzzle in survival games.

I played at release too but that never happened to me. You could always hear where it was in the vents, and spot the traps by being cautious
Why bullshit? I finished the game on the hardest difficulty and as long as you didn't do stupid shit you would be fine.
