For me it was an unsecured data port in a testing room, on a LAN leg that didn't have loop back protection. Spare cable in the room. One of the new hires plugs port 96 into port 97. Cue gigantic as fuck broadcast storm. Took the SAN array offline, which took down the vSphere data center and 150+ remote clients. Took 2 hours to take each of the other segments offline, add them back in one at a time. The stupid sales trainer installed a 8 port HUB (HUB!) for those ports and never told IT about it.
